Utah Jazz Donation Requests

The Utah Jazz proudly supports charitable organizations and fundraisers through donations of merchandise. If you would like to request a donation, please fill out the form below. Click here to read our donation guidelines.

Requests should include an official donation request letter on the organization's/association's letterhead. All letters should include the following:

• Brief description of the event (including date and location)
• Beneficiary of proceeds
• Charitable registration number (if applicable)
• Contact Name, Address, Phone Number & Email

SUBMISSION CRITERIA

The information below details Utah Jazz donation guidelines. Prior to completing your submission, please read and review the following:

• Only one donation request per organization, per calendar year may be submitted.
• Donation requests must be received six weeks prior to the event date (NO EXCEPTIONS).
• Your event must benefit a charity or non-profit organization. Events supporting individuals will not be considered.
• The organization/association requesting the donation must be located within Utah, Idaho, or Wyoming.
• Monetary donations, sponsorships, door prizes, or giveaways will not be considered.

NOTE: All requests must be submitted through the online donation system. However, submitting a request does not guarantee a donation.
